Jujutsu Kaisen episode 26 reveals a lot of small, but impactful information about its characters. One of the most recent revelations from this episode is Gojo's brain condition during the fight against Sukuna. It is undeniable that it was the most difficult battle of Gojo's life and the result clearly showed the strength of the two strongest spellcasters of the past and present.
An illustration in episode 26 depicts Gojo's brain bleeding profusely, due to overexertion. His brain condition was so bad that it was a wonder how he was able to continue fighting and sparring with Sukuna.
For Gojo, maintaining his mental state is more important than for any other character. That's because his Limitless Technique puts too much stress on his brain. But he continued to use the Reverse Curse to relieve stress and maintain a healthy condition.

However, when Sukuna broke Gojo's domain expansion ability, he could no longer use his Reverse Curse technique, resulting in his brain being stressed. Furthermore, he became more and more tired. These two things together caused severe damage to Gojo's brain.
Gojo's brain condition and his internal pain were alluded to several times during their fight, most notably when he began bleeding from his nose.
In Jujutsu Kaisen episode 26, Gege Akutami finally showed fans how seriously damaged Gojo really was in this fight. The picture of his bleeding brain was extremely bad. Fans were horrified to find this information about their favorite character and condemned the author's hatred for the character.
Fans also pointed out that Sukuna had bleeding eyes during the fight with Gojo and must have suffered internal bleeding as well.
