After letting Kenshin stay with him, the doctor sent her a text message. Later in the episode, we see Ishizu plotting to deal with the doctor, which makes it clear that Ishizu is using the strange man from before to deal with Kenshin. The man reveals at the end of the episode that he really wants to fight Kenshin and that he knows who he is.
