After the recent episode of Jujutsu Kaisen Season 2, fans were confused by Sukuna’s feelings for Megumi, as he pulled Megumi out of a “hanging death” situation even though he was Sukuna’s enemy.

After Megumi summoned Mahoraga in episode 17, no one could defeat this monster.
But Sukuna suddenly appeared and saved Megumi. He first heals Megumi with his curse reversal technique and then defeats Mahoraga.
After defeating this monster, he released Megumi near Ieri so that Ieri could be fully healed. This clearly shows that he doesn’t want Megumi to die.
Megumi and Sukuna: The Origin
In the terrifying Wonb Arc, Sukuna and Megumi fight each other for the first time. Since this arc, Sukuna has developed a strange interest in Megumi Fushigoro, which has not been made clear in the anime.
Megumi intended to summon Mahoraga for the first time in this fight but avoided it as Itadori regained control of his body.
But the cursed energy he emitted when summoning Mahoraga piqued Sukuna’s interest to the point that he began following Megumi in the future.

Why did Sukuna save Megumi’s life in Episode 17?
Sukuna saves Megumi from being killed because he wants to make Fushigoro Megumi his future host after leaving Itadori’s body.
As we know, Sukuna’s true power has not yet been revealed because inside Itadori’s body, he has no voluntary control over his host due to Yuji’s extreme will.
Additionally, Itadori does not have a natural curse technique (like Megumi’s Ten Shadows Technique). Therefore, changing the host from Itadori to Megumi will give Sukuna more space to express his power.

When did Sukuna accept Megumi as a Host?
Sukuna changed host from Itadori to Megumi during the Culling Games Arc. The extermination game is an event organized by Kenjaku in which witches compete against each other.
At the end of the Shibuya Arc, Kenjaku performs an idle transformation on non-witches near him to enter them into the Extermination Game.
Tsumiki was also transformed into a participant in the Extermination Game during this time. So Itadori and Megumi joined this game to save Tsumiki.
But unfortunately, a cursed soul was reincarnated with Tsumiki’s body, named Yorozu. When this is revealed, Itadori and Megumi are confused as to what happened.
Sukuna took this opportunity to activate his Oath of Binding (meaning taking over Itadori for a few minutes, with the promise not to hurt anyone except him) on Itadori and made Megumi eat one of the his fingers, growing from Itadori’s body.
Soon after, Sukuna took over Megumi’s body as his new host. He then crushed Megumi’s will by killing his sister with his body so that he could no longer take over Megumi’s body, just like Itadori.
