While these theories may have arisen due to Yuji and Sukuna's physical similarities, several interesting subsequent plot developments in the manga have been offered that further hint at a connection between two people.
The fact that Kenjaku was involved in Itadori's birth and that Sukuna certainly recognized Itadori as “the boy from back then” has reinforced the speculations and made fans wonder if these two have any connection in the past. past or not. The possibility of Itadori being Sukuna's reincarnation was also raised.
And, it seems like those speculations weren't wrong after all. In JJK chapter 257, we finally get some of the answers we've been waiting for!

What is Itadori's relationship with Sukuna?
To answer in short, Itadori is Sukuna's grandson!! Itadori is related to Sukuna through his father, Jin Itadori, who is the reincarnation of Sukuna's twin brother.

Now, the twin theory is something completely different. However, that affected the relationship between Itadori and Sukuna.
When it was revealed that Kenjaku was Itadori's mother, it sent the fan community into a frenzy. However, the question arises as to why the flamboyant schemer chose Jin Itadori as Yuji's parents.
Yuji was definitely not created on a whim. Knowing Kenjaku, Yuji is the culmination of years of research into optimizing cursed energy and surpassing human potential. Therefore, Jin must have been deliberately chosen by Kenjaku.
But why? Is it his genes? Or his cursed technique?
These questions have been begging to be answered for a long time. And in chapter 257, we finally get a better understanding of Jin's conundrum.
According to what Sukuna said in the chapter, Jin Itadori is the reincarnation of Sukuna's twin brother, the twin brother he consumed/absorbed while they were in the womb.

We don't really know how this particular reincarnation took place, but we can draw some conclusions.
Since jujutsu treats the twins as a single entity, the soul of Sukuna's other half was presumably not extinguished even if the body was consumed in the womb. Similar to how it was hinted that Mai's soul remained in the sword she created for Maki, Sukuna's twin brother's soul went through many cycles and was eventually reincarnated as Jin Itadori.
Kenjaku probably realized this and decided to use Jin Itadori, and the essence of the Sukuna twins within him, to create/give birth to Yuji.
So that makes Sukuna Yuji's uncle, technically speaking!
When you first read this development, it may feel like a retcon. However, that is not the case. This development, and specifically the fact that Jin Itadori played an important role in Itadori's birth, is hinted at in the manga.
Warning for the present time:
In chapter 1, when Yuji left to visit his grandfather, his grandfather had the last words to say to him. And as Wasuke Itadori mentioned, it involves Yuji's parents. But Yuji ignored his grandfather and we never knew what he wanted to say.

But now, I guess we can make some educated guesses.
In chapter 143, we see that Wasuke was wary of Kaori Itadori, as she was possessed by Kenjaku. However, Wasuke didn't just want to talk about Kaori, but also about his parents.
I believe that Wasuke's dialogue in chapter 1 was the first hint Gege gave us, showing that both his parents have important roles in the JJK universe.
On top of that, we still don't know what happened to Jin Itadori. Is he dead? Did Kenjaku kill him? We may get answers to these questions in the near future.
The second important hint we get is from Sukuna himself as I mentioned earlier in the article. In chapter 214, after witnessing Itadori's display of raw power, Sukuna notes that “the boy is from that time” and then goes on to say that Kenjaku does the most disgusting things.
I guess this is when Sukuna first figured out Kenjaku's plans and noticed Itadori was related to him through his twin brother! However, for us readers, this is the second big hint that Yuji has a relationship with Sukuna and that Kenjaku did something truly evil in his creation.
We still don't fully understand However, as for why Kenjaku created Itadori, I guess that will also be revealed soon, even though he appears to be dead for now.
