During the final battle, Shigaraki's body began to mutate, searching for a new form more suited to using the immense power hidden within him.

However, Tetsuo is consumed by his own power and transformed into a hideous creature, requiring the intervention of Akira, another psychic who has transcended mortal form.

Ragyo Kiryuin is the evil mastermind of Kill la Kill, a being experimented on using alien Life Fibers, which eventually took over almost her entire body. Ragyo is functionally immortal, only able to be killed if every Life Fiber in her body is severed, something only scissors can do.. Anything less than a critical hit is useless. She has tremendous strength, being able to easily defeat her daughter Satsuki even while wearing Kamui (a costume enhanced with Life Fibers). Ragyo even calls herself the Ultimate Life Form in the series, mocking the heroes.
Ragyo's power could only be stopped by Ryuko having similar powers, and even then it was still a difficult battle, as Ragyo had more experience.

Kars is a Pillar Man, a creature far more powerful than any human, but he reached his Ultimate Lifeform state after wearing a Stone Mask equipped with the Aja Redstone. Kars' abilities became hundreds of times better than before, allowing him to quickly surpass Joseph, and his previous weakness to sunlight disappeared, rendering Joseph's Hamon useless.. He can mutate his body at will and regenerate from any wound, no matter how severe. Joseph's defeat of Kars was purely by luck, he pushed Kars out of the atmosphere through a volcanic eruption.
Kars's perfect survival instincts eventually backfired on him, as he was trapped in deep space, unable to move or escape forever. It is said that his mind eventually shut down due to boredom, but he still floated in orbit, technically alive. This left Kars certain that he was Ultimate Lifeform Best appeared in anime.
