This supernatural ability drives all of the main events of the manga, especially in the later chapters, and would still be effective even if Lady Eleanor were not the villain and did not receive isekai. Before the events of the manga, her telepathy was more of a curse than a gift, as most of the thoughts she could read were negatively directed at her. It clearly took an emotional toll on Lady Eleanor and left her physically exhausted. However, when the prince officially appears, previous knowledge of Lady Eleanor’s negative psychic experience enhances their relationship, as his thoughts reveal him to be the man perfect and think the right things.
Of course, things get more complicated when the thoughts of random people inform her of an impending tragedy that could put her relationship with the prince in jeopardy, so she try to take advantage of what you can naturally hear to stop what seems to be happening. Ironically, telepathy in this case serves as a substitute for the pre-existing knowledge that most isekai protagonists have. Lady Eleanor’s powers are causing her to know information she shouldn’t know, as telepathy is not a normal ability in this world. This functions similar to how an isekai protagonist uses their experience while playing the game in which they receive isekai for their own benefit. It is therefore reasonable that Lady Eleanor had little or no knowledge of the game in which she participated. Similarly, there are cases where Lady Eleanor manipulates those around her by saying things out loud that contradict her target’s thoughts instead of relying on her previous knowledge of the game.
Although there is a villain who gets transported into an otome game and there is an isekai plot twist later on, this is not what makes the series so engaging.
